On the CORE One equipped with the INDX, the offset of the tools compared to the first is calibrated through the Tool Offset Calibration. If the offset of one of the tools deviates too far from the expected range, the printer will show the following message: "Tool offset calibration failed. Make sure the nozzle and the sensor are clean. Caution: the nozzle may be hot."
Error name: Tool offset out of bounds
Error code: #36130 (CORE One INDX)
Make sure that the filament is unloaded before the calibration process.
Use a small brass brush to clear away any old plastic stuck on the nozzle. Avoid contact of the brush with the hotend cables, as this could cause a short circuit.
Go to the printer menu in Control -> Calibrations & Tests -> 12. Tool offsets calibration and redo the calibration.
Any vibration propagated to the surface under the printer, or an unstable surface under the printer, might cause a false reading on the load cell, triggering the homing error.
For example, a printer that is in operation next to your printer can cause vibrations that are propagated to it, potentially causing a tool offset calibration failure. In that case, stop any printers next to the CORE One with INDX, and reattempt the calibration.
A possible solution to dampen the noise and vibration of a printer is to add some additional padding underneath the printer.

I AM PRINTING SUCCESSFULLY NOW. The problem appears to be electromagnetic interference, most likely for me between the bed heater wires and the INDX tool head cable. Bed heats, tool offset calibration wanders around lost.
During installation I had done some nice cable management and apparently created a coil in the main cable that was introducing the EMI. Popped the back cover, stretched out the cable and have been printing fine since.
You can find more about this on the Prusa Discord by searching for “ferrite core”. Another way to combat this interference is to install a ferrite core around the INDX main tool head cable close to where it plugs into the Buddy Board.
Happy printing!
Even stranger, during the calibration, some of the tool heads are probing the sensor on the front right corner. That can't be good for the board. Hopefully they have a better guide to troubleshoot...
When Printing, the nozzle temperature is higher than during calibration, and it appears that there is a slight amount of filament dripping from the loaded filament... but it's really just a tiny amount. I tried using both Prusament PLA and
ESUN PLA, but both yielded the same result. Since performing the INDX conversion, I haven't been able to print a single time.
please try to use 6.9.1 beta (or stable if released by the time you are reading this reply). One of the changes we did there is redusing the nozzle temperature for the tool offset calibration.
You can find the file here: https://github.com/prusa3d/Prusa-Firmware-Buddy/releases/tag/v6.9.1-beta
Unsafe Z tool offset #36131
Update:
after installing the new driver I ran the new gantry squareness wizard and it found a skew, I went back through this and belt tuning twice (and learned reverse belt tuning is bad)
no luck
so i did all the calibrations again and they all passed
still getting the same error
Update 2:
I finally got it to print...with no changes since the last update, just slicing and sending again....shrug
